Persistent trigeminal artery combined with cerebral infarction

Background Persistent trigeminal artery(PTA) is a rare embryonic connection between the cavernous part of the internal carotid artery and the basilar artery. The clinical significance of PTA remained unknown. We experienced one case of PTA case and reviewed the literature of PTA cases. Case Report A 57-years old woman presented with left sided paresthesia. She had the history of hypertension and diabetes. MR scans showed an acute right lateral medullary infarct and left PTA. Digital subtraction angiography revealed left PTA with medial type, Saltzman type 1. Conclusion PTA is rare phenomenon and often associated with intracranial aneurysm, arteriovenous malformations, carotid cavernous fistula, Moyamoya and cerebral ischemia. We reviewed 11 cases from the previous articles and eight cases had the cerebral infarction. Seven subjects had pontine infarction and one had medullary infarction. Among the patients with PTA and cerebral infarction, six patients had Saltzman type 1 and 4 patients had median type. This results suggested that PTA, Saltzman type 1 might be associated with brainstem infarction. Further study should be needed to clarify the association between stroke and PTA subtypes.
