일산화탄소 중독에 의한 양측 대퇴신경병증 - 증례보고 -
초록
Carbon monoxide (CO) is a infrequent cause of morbidity due to poisoning, Simultaneous bilateral femoral neuropathies are very uncommon and have been reported as results of pelvic surgery or related to pregnancy. We have experienced a patient with bilateral femoral neuropathy and rhabdomyolysis associated with CO poisoning. A 72-year-old male was admitted to hospital in a deep coma followed by CO poisoning. The patient has been found lying on bed with hip extended position. Laboratory results showed rhabdomylosis (total creatinine phosphokinase, 18,260 IU/L; CK-MB, 80.2 IU/L) and concentration of CO-Hb was 17.5%. After receiving oxygen therapy, the patient regained consciousness; however, he cannot walk due to bilateral knee extensors weakness. Nerve conduction and electromyography revealed symmetrical femoral and saphenous neuropathy. Pelvic plexus magnetic resonance imaging (MRI) was performed and revealed rhabdomyolysis with both femoral nerve injuries at the confluent portion of iliacus – psoas muscles. After taking the rehabilitation programs, the patient achieved functional improvement in ambulation, endurance and balance.
