Fine-needle aspiration cytology of spindle cell liposarcoma: A case report

초록

Liposarcoma is the second most common soft tissue sarcoma in adulthood comprising approximately 20% of cases. Among them, spindle cell liposarcoma is an uncommon variant of well-differentiated liposarcoma. It occurs in middle-aged adults with a male predominance. Its locations include the limbs, buttocks, trunk, head and neck, posterior mediastinum, larynx and pharynx. It is a low-grade sarcoma capable of recurrence, dedifferentiation, and metastasis. We report on the fine-needle aspiration (FNA) cytologic findings of a well-differentiated spindle cell liposarcoma in the posterior neck from a 70-year-old male. Cytologically, moderately cellular smear predominantly composed of spindle cells. Some areas reveal fine arborising vessels and spindle cells in the myxoid background. Additionally, atypical spindle cells are found between mature adipocytes. Lipoblast-like cells are also present. Spindle cells are relatively bland and arranged in short fascicles. They show mild pleomorphism and a few mitoses. Cell border is indistinct and cytoplasm is scanty. The nuclei are hyperchromatic with distinct nucleoli. An excision for mass was performed. Grossly, the specimen was an ovoid shaped soft tissue mass with irregular margin, measuring 5.0x3.0x3.0cm. The cut surface was yellowish gray and myxoid. Microscopically, the tumor showed hypocellular myxoid or hyaline areas alternating with areas showing greater cellularity. The tumor mainly consisted of relatively bland spindle cells arranged in short fascicles or whorls with patternless, moderately rich capillary network.
